EM116: Workshop: The indivual, inclusive and contextualised approach of the patient with severe acute brain injury and rehabilitative continuation - Bobath Concept

Introduction and course aims:
The Bobath concept is a problem solving approach aimed at the evaluation and treatment of people with movement disorders, postural control and function, caused by a central nervous system injury. This approach to adult rehabilitation with central neurological damage originates from the work of Berta and Karel Bobath and has evolved over the last 60 years. The rationale for its application has its roots in current neuroscientific knowledge related to motor control, motor learning, neural and muscular plasticity and current biomechanical knowledge. The aim of the course is to deepen the knowledge of the specific problems of the patient with severe head trauma for a better evaluation ability of the different clinical pictures, it is to develop the observation capacity and the clinical reasoning to identify the aspects that limit the functional recovery, is to implement thetechnical skills for the treatment in order to better cope with the critical aspects highlighted. The structure of the workshop-like event will leave ample space for the presentation of clinical cases and the practice / laboratory for the development of the manual skills of the operators.
Specific objectives:
- Promote evidence-based approach based on updated literature
- Acquire specific terminology to describe the rehabilitative approach to the brain injured patient according to the Bobath Concept
- Updating knowledge in the biomechanical and neurophysiological fields in relation to the problems of the patient with severe brain injury in the different phases of the rehabilitation process
- Refine the ability to monitor and acquire models for the evaluation of acute, post-acute and chronic patients
- Implement treatment techniques to encourage the recovery of brain injured patient within their daily life context
- Refine to dexterity and implement technical competence at an introductory level
- Support comparison and discussion between participants and clinical reasoning

8.30-9.00 Registration of participants and presentation of the objectives of the course
9.00-11.00 Introductory lecture on the subject of the course: "Treatment of the patient with severe head trauma in the different phases of remission after persistent vegetative state"
11.00-11.15 Coffee break
11.15-12.45 Demonstration of the patient A by the teacher in a collective session (severe head trauma in the vegetative state or in the initial phase of remission)
12.45-13.15 Synthesis of clinical reasoning and discussion
13.15-14.15 Lunch
14.15-15.30 Positioning of the patient after treatment (video, images and practical laboratory)
15.30-15.45 Coffee break
15.45- 17.30 Practical laboratory: Potential and limits of the supine position, regulation of the tone of the trunk for the activation of the core stability. Enhancement of postural control in the lower trunk to free the hips.
17.30-17.45 Summary of the day's learning and conclusions.
Second day
8.30-11.00 Lesson: "Treatment of impairment / compensation treatment: comparisons with literature and clinical experience"
11.00-11.15 Coffee break
11.15-12.45 Demonstration of patient B by the teacher in a collective session (severe or moderate head injury in the most advanced phase of remission)
12.45-13.15 Synthesis of clinical reasoning and discussion
13.15-14.15 Lunch
14.15-15.30 Practical laboratory: Stability of the head and activation of the upper trunk in rotation on a base of sealing of the lower trunk
15.30-15.45 Coffee break
15.45- 17.30 Practice workshop: Rotation of the upper trunk on a stable lower limbs base - Roll towards the sitting position.
17.30-17.45 Summary of the day's learning and conclusions.
Third day
8.30-11.00 Lesson: "The concept of the 24-hour approach - home care and the self-employment program"
11.00-11.15 Coffee break
11.15-12.45 Demonstration of the patient B or C by the teacher in a collective session (severe or moderate head injury in the most advanced phase of remission)
12.45-13.15 Synthesis of clinical reasoning and discussion
13.15-14.15 Lunch
14.15-16.00 Practical laboratory: Sit-to-stand with various specific facilitation techniques for the patient with head injury. Train to verticality.
4.00 pm - 4.30 pm ECM test, delivery of certificates and completion of the course
